Privacy policy

This privacy policy describes how Oaks Oral Surgery & Periodontics (“we”) collects and uses information through this website. Use of patient health information in clinical care is governed by our separate Notice of Privacy Practices, provided to every patient at their first visit.

When you submit the callback request form, we collect your name, phone number, email, patient type, preferred contact time, and contact method. The form is intentionally designed to avoid collecting medical history, symptoms, or other protected health information.

When a referring doctor uses the secure online referral form, we collect the referring doctor’s contact information together with the patient information needed to schedule and prepare for treatment. This transmission is permitted under the HIPAA treatment exception (45 CFR § 164.506).

We collect basic web analytics (page views, referring URLs) to understand how the site is used. We do not use third-party advertising trackers or sell visitor data.

Form submissions are routed to authorized practice staff for the sole purpose of responding, scheduling, and providing care. Staff access to records containing patient health information is logged.

Form data is stored in an encrypted database hosted on infrastructure that supports a HIPAA Business Associate Agreement. File attachments uploaded with referrals are stored encrypted at rest and transmitted over TLS. Access is restricted to authorized practice personnel.

Standard email is not a secure channel for protected health information. Please do not include medical history, symptoms, or images in email correspondence with the practice. Use the secure referral form, the patient portal, or call us directly.

You may request access to, correction of, or deletion of personal information you have submitted to us, subject to record-retention obligations under applicable health-care law. To exercise these rights, contact us by phone or mail using the information below.
